There are two ways to access your protected health information.
SLOW METHOD
Download the Patient Request Form, and return it to us via:

Email a photo of your form to sidspharmacy@sidspharmacy.com
FAST METHOD
Create an account in our Prescription Portal. You then have immediate access to your profile. Print out your tax information directly from the “Profiles” tab.
